  • Current Lease Agreement, Lease Purchase Agreement or Lease Amendment(s) and / or any Inter‑Governmental Agreements, Memorandums of Understanding or Joint Powers Agreements

  • Applicable, Certification(s) and / or Lease Amendment:

    • Certification A - Public Facility

    • Certification B - No Public Facility Available

    • Certification C - Facility Owner - Non-Profit Foundation

    • Lease Amendment - Owner Maintains Facility to Statewide Adequacy Standards

  • “E” Occupancy Certificate

  • PSCOC’s Annual Conflict of Interest Questionnaire signed by Governing Board

  • District’s or charter school’s Conflict of Interest Policy

  • PSFA's Notification of Net Square Footage Assessment Letter
